your physician. For more complex situations, you may wish to contact our office manager.
To arrange an appointment, please call (732) 530-1515. Our staff will make every effort to
schedule your appointment as soon as possible. If you are unable to keep your scheduled
appointment, please give us at least 24 hours' notice. Since this is a trauma practice, we may
occasionally have to reschedule your appointment to handle an emergency. In the event that
you need emergency care, one of our doctors is always on call.
All of our offices (Tinton Falls, Lakewood, and Manahawkin, NJ) are open Monday through Friday
between the hours of 9:00 a.m. and 5:00 p.m.
We request that fees for office visits be paid at the time service is rendered. At that time, we'll
provide you with a receipt that you can attach to your insurance form for submission. For
larger amounts such as surgical fees, our office manager will work with you to arrange a
payment plan that fits your budget.
Insurance Claims
Our office will file your insurance claims for major procedures. However, YOU MAINTAIN
FINANCIAL RESPONSIBILITY FOR ALL SERVICES RENDERED. If you have
questions about reimbursement, we're always available for consultation.
Medicare
Patients will be responsible for the 20% co-ins as well as their yearly deductible.
Motor Vehicle and Worker's Compensation
In order for our office to submit your claims for you, you must provide us with the following
information on your first visit: insurance company name, address and telephone number, as
well as your claim number, date of accident and claims adjuster's name. If you do not have
this information available, you must either assume responsibility for payment or reschedule
your appointment. In the event that your insurance declines or terminates your benefits, you
